- Overview
This Privacy Policy explains how Lindsay Meldrum, carrying on business as Bare Laser & Skin ("Bare Laser & Skin," "we," "us," or "our"), collects, uses, discloses, and protects personal information when you visit www.barelaserskin.ca, use affiliated booking tools, contact us, visit our London clinic, or otherwise interact with us.
We handle personal information in accordance with Ontario's Personal Health Information Protection Act, 2004 (PHIPA) and, where applicable, PIPEDA. Information about your health, assessments, treatments, and payment is personal health information and is maintained as a confidential health record. We may update this policy and will revise the date above.
2. Our Ten Privacy Commitments
We follow fair information principles: accountability; identifying purposes; consent; limiting collection; limiting use, disclosure, and retention; accuracy; safeguards; openness; individual access; and the ability to challenge compliance by contacting our Privacy Officer.
3. Information We Collect
Information Collected Automatically. We collect Device Information such as IP address, browser and operating system, time zone, referring pages, pages viewed, and visit date and time. We use cookies, log files, web beacons, tags, and pixels for these purposes.
Information You Provide. We collect information you give us through contact forms, consultation requests, mailing lists, text messages, promotions, reviews, social media, email, and phone calls, including name, email, phone number, address, inquiry, and treatment interests. Please do not send detailed medical information through unsecured channels.
Information Collected at Our Clinic. For appointments and treatment we may collect contact details, intake and health history, medications and allergies, assessments, treatment records, photographs, consents, and payment records. If necessary information is declined, we may not be able to treat you.
Information From Other Sources. We may receive information from social platforms, advertising and analytics partners, referrals, vendors, and financing providers. Personal information refers collectively to all of the above.
4. How We Use Your Information
We use personal information to respond to inquiries; schedule and follow up on appointments; assess suitability and provide care; maintain client records; process payments and memberships; administer promotions; send consent-based marketing; operate and improve the Site; measure performance; prevent fraud; and meet legal, regulatory, professional, insurance, and recordkeeping obligations. We do not sell personal information or use health information for marketing without separate express consent.
5. Consent
We identify purposes and obtain consent before or when collecting information. For sensitive health information we seek express consent. Law may permit collection, use, or disclosure without consent for emergencies, court orders, legal proceedings, or other reasons permitted by PHIPA and PIPEDA. You may withdraw consent subject to legal restrictions and reasonable notice; clinical records must still be retained as required by law.
6. How We Share Your Information
We share information with service providers such as our host, booking platform, payment processors, messaging providers, analytics providers, and marketing partners, who must protect it and use it only for specified purposes. Payment card details are processed by our processor. With consent, we may share relevant information with other health professionals. We may disclose information when required by law, for safety, business transfers, or in aggregated and de-identified form.
7. Storage Outside Canada
Some providers store or process data outside Canada, including in the United States, where it may be accessible under that country's laws. By providing personal information, you consent to this transfer and storage. Contact our Privacy Officer with questions.
8. Photographs And Images
Clinical photographs form part of your client record and are protected as personal health information. We will not use photographs, video, or likeness in marketing, social media, our website, or education without separate express written consent. Consent may be withdrawn in writing, though withdrawal does not reach materials already published.
9. Reviews And Testimonials
Public reviews are visible to anyone, including health or treatment details you choose to include. Confirming that a reviewer is a client can itself disclose personal health information, so please consider carefully before posting health details.
10. Cookies And Tracking Technologies
You can set your browser to remove or reject cookies, though some Site features may not work. We do not currently alter data collection in response to Do Not Track. Interest-based advertising opt-outs are available through the Digital Advertising Alliance of Canada at youradchoices.ca.
11. Marketing Communications And CASL
We send commercial electronic messages in accordance with Canada's Anti-Spam Legislation, relying on express or permitted implied consent. Messages identify us, provide contact information, and include an unsubscribe mechanism. Reply STOP to texts or contact us to withdraw marketing consent; transactional appointment messages may continue.
12. Children And Minors
The Site is not directed to children and we do not knowingly collect personal information online from anyone under eighteen. Where Services are provided to a minor, the same protections apply and consent is obtained as appropriate. Contact our Privacy Officer if a minor has provided information.
13. Safeguards
We use administrative, technical, and physical safeguards appropriate to sensitivity, including restricted access, confidentiality obligations, privacy training, secure clinical records, and access controls. No internet transmission or electronic storage is completely secure. We will notify affected individuals and regulators of a qualifying breach where required.
14. Retention
We retain clinical records for the period required by law, professional standards, and our insurer. Marketing and website information is retained as needed. When information is no longer required, we destroy, erase, or de-identify it securely.
15. Your Rights
Under PHIPA and PIPEDA you may access personal information we hold and information about its use and disclosure; correct inaccurate information; withdraw consent subject to restrictions; and challenge compliance. Contact our Privacy Officer. We generally respond within thirty days, may verify identity, and will explain any lawful refusal. A reasonable fee may apply where permitted.
16. Third Party Links And Social Media
Third parties operate independently, may collect information, and have their own privacy policies. We are not responsible for their practices. Review third-party policies before providing information.
17. Changes To This Policy
We may update this Privacy Policy for operational, legal, or regulatory reasons. The revised policy will be posted here with an updated date.
